Direct Support Professional

The Direct Support Professionals shall be responsible for implementing the day-to-day needs of each individual receiving support. Direct Support Professionals shall provide learning and support for individuals who have an intellectual disability in both residential and community-based environments. Direct Support Professionals will also assist with the implementation of Individual Person-Centered Plans, and provide learning and support in areas of domestic skill-building, community skill-building, money management, personal care, community integration activities and behavior intervention, and housekeeping.

Community Support Professional (QPPMH)

The Community Support Professionals are responsible for the day-to-day implementation of each individual’s ISP. Community Support Professionals will be responsible for providing individuals with quality and professional services. Community Support Professionals will support individuals in skill-building areas and have a general knowledge of the Office of Human Rights and DMAS Regulatory Guidelines.

MAJOR DUTIES:

  • Provide supervision and support for individuals receiving mental health supports.
  • Adhere to State licensure and human rights regulations and guidelines.
  • Maintain current, accurate, and appropriate individual records and other relevant documentation involving individuals and service delivery.  
  • Provide the proper behavioral assistance and intervention for individuals.
  • Attend Quarterly and Annual Review Meetings and agree to participate and provide input regarding the provision of supports for individuals receiving service.
  • Maintain and renew all First Aid and CPR training and other required certifications necessary to carry out program services.
  • Maintain quality health and safety management standards to protect the health, safety, and welfare of individuals.
  • Adhere to all Heart & Soul LLC “COMMUNITY EXCURSIONS” program policies and procedures
  • Maintain knowledge of current services by attending workshops/seminars/conferences
  • Assist individuals with support needs according to their ISP.
  • Maintain confidentiality and security of individual service records
  • Daily Documenting in individuals Progress Notes

Part-Time Shifts available

Required License or Certifications or Qualifications

“Qualified Paraprofessional in Mental Health (QPPMH)” means a person who must, at a minimum, meet one of the following criteria:

  • Registered with the United States Psychiatric Association (USPRA) as an Associate Psychiatric Rehabilitation Provider (APRP);
  • Has an associate’s degree in a related field (social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling) and at least one year of experience providing direct services to individuals with a diagnosis of mental illness; or
  • Has a minimum of 90 hours of classroom training and 12 weeks of experience under the direct personal supervision of a QMHP-Adult providing services to individuals with mental illness and at least one year of experience (including the 12 weeks of supervised experience).

KNOWLEDGE OF:

SKILLS IN:

  • Supervising individuals with a wide variety of support needs

Community Support Professionals II (QMHP-A)

The Community Support Professionals II are responsible for the day-to-day implementation of each individual’s ISP. Community Support Professionals II will be responsible for providing individuals with quality and professional services. Community Support Professionals II will support individuals in skill-building areas and be knowledgeable of the office Magellan, DBHDS, the Office of Human Rights, and DMAS Regulatory Guidelines. The Community Support Professionals II will be supervised by the LMHP and Program Administrator.

MAJOR DUTIES:

  • Provide supervision and support for individuals receiving mental supports.
  • Adhere to State licensure and human rights regulations and guidelines.
  • Maintain current, accurate, and appropriate individual records and other relevant documentation involving individuals and service delivery.  
  • Provide the proper behavioral assistance and intervention for individuals.
  • Treat all individuals receiving support with dignity and respect at all times and be informed that you, as an employee providing a provision of services, are a mandated reporter of any Human Rights Violation.
  • Attend Quarterly and Annual Review Meetings and agree to participate and provide input regarding the provision of supports for individuals receiving service.
  • Maintain and renew all First Aid and CPR training and other required certifications necessary to carry out program services.
  • Maintain quality health and safety management standards to protect the health, safety, and welfare of individuals.

QUALIFICATIONS:

  • The LMHP, LMHP Supervisee, or Resident will supervise the care if delivered by the  QMHP-A.
  • Valid operator’s driving license
  • Satisfactory Criminal Records Report and Child Registry Background Check
  • TB-Test with results of not having TB in a contagious form.
