Heart & Soul ALF offers a safe, wholesome and homelike environment for ambulatory residents. We are staffed with qualified professionals, who are CPR/First Aide certified. Staff is on duty 24hrs a day, 7 days a week. There is a Licensed Practical Nurse (LPN) on duty four (4) days a week in case of minor non-life threatening emergencies. Client/Residents are provided three (3) nutritious meals and three (3) snacks per day. We also provide activities/outings for all who are interested.

Day Programs and Workshops are strongly recommended, and transportation will be arranged. Facility outings transportation will be provided., and most importantly we meet individuals at their level of functioning and we’re making a difference!

Population Characteristics & Needs of Population Served:

• Males and females 18 years of age and above
• Mentally challenged and/or mentally ill adults with history of substance abuse
• Individuals who are ambulatory
• Individuals who are visually impaired and / or hearing impaired.
  

Admittance Criteria

• Based on Diagnosis
• No incontinent (Case by Case)
• Ability to go to Day Support Program
• No history of Aggression with-in last 5yrs (Case by Case)

Services resident are entitled to are as follows:

• Semi-Private + Quad Room
• 3 meals & 3 Snacks
• Medication Management
• Mild Assistances with ADL’s
• Setting up Doctor Appointments
• 24-hour Supervision by qualified DCS/RMA

TIER 1 Auxiliary Grant Pay Rate + $10 Laundry fee (see disclosure statement)

• Basic Rate is determined by the Virginia General Assembly and is adjusted periodically.

TIER 2 Private Pay Rate of $1300 – $3499 (see disclosure statement)

• Private pay rate is based on resident’s income.
