Heart & Soul I Founded

Heart & Soul I Assisted Living Facility was founded with the vision of creating safe, supportive, and compassionate environments for individuals needing care and community support.

1994

Heart & Soul II Opens

Opening of Heart & Soul II Assisted Living Facility providing compassionate residential care and support services for elderly individuals and those with intellectual disabilities.

June 2001

Heart & Soul III Expands

Expansion and enhancement of Heart & Soul III Assisted Living Facility services with a continued focus on dignity, family-centered care, compassion, and belonging.

Oct 2009

Crispell Group Home Established

Establishment of Crispell Group Home to support individuals with intellectual and developmental disabilities within a structured residential environment promoting independence.

Sept 2011

Community Excursions PSR Created

Creation of Community Excursions Psychosocial Rehabilitation (PSR) program to support independence, recovery, social reintegration, and community engagement for individuals with serious mental health challenges.

Feb 2017

M.M.B.R. Skill Building Launches

Launch of M.M.B.R. Skill Building Services focused on empowering individuals living with serious mental illness through mental health skill-building, life-skills development, and community stability supports.

Jan 2023

Connections Sponsorship Formed

Formation of Connections Sponsorship with the first location established in Richmond, Virginia, to strengthen supportive residential opportunities and person-centered support services for individuals with IDD.
