Safe, Supportive Residential Living That Promotes Independence
Heart & Soul LLC provides Residential Living in a safe, welcoming, and person-centered home environment for adults who benefit from daily structure, supervision, and individualized support. Our Residential Living program is designed to help individuals live as independently as possible while enjoying the comfort of a stable home and meaningful participation in their community.
Unlike Assisted Living Facilities, our Residential Living program does not provide skilled nursing care, ongoing medical treatment, or clinical healthcare services. Instead, we provide housing, meals, supervision, assistance with daily living activities, and personalized support while residents continue receiving medical, behavioral health, and other clinical services from their own licensed healthcare providers.
Our goal is to create an environment where every individual feels respected, valued, safe, and empowered to achieve greater independence and an improved quality of life.

We encourage residents to engage in recreational activities, volunteer opportunities, social events, and other community experiences that promote inclusion, confidence, and meaningful relationships.
Life Skills Development
Residents are encouraged to strengthen practical life skills such as personal hygiene, household responsibilities, communication, decision-making, budgeting, and healthy daily routines to increase independence.
Wellness Monitoring
Our staff monitor each resident's overall well-being and communicate with families, guardians, support coordinators, and healthcare providers when appropriate to ensure continuity of support.
Transportation Coordination
Heart & Soul LLC assists individuals with coordinating and scheduling transportation services available through their Medicaid or insurance benefits when applicable, helping residents access medical appointments, community services, and other approved activities.
Individualized Support Planning
Each resident receives a person-centered support plan that reflects their goals, preferences, strengths, and evolving support needs while encouraging greater independence and community involvement.
