The Program Administrator or Program Administrator Designee of Heart & Soul shall ensure its quality of services. The Program Administrator or Administrator Designee is responsible for the day-to-day direction of Heart & Soul’s Medicaid-funded programs. Heart & Soul LLC shall provide supervision and evaluation of all program supports by conducting scheduled and unscheduled reviews.
Heart & Soul utilizes “The DMAIC Process” to define organizational areas that need improvement. The goal remains focused on delivering our individuals the best care possible through quality initiatives and continuous quality improvement.
Quality assurance evaluations shall be based upon current DBHDS policies and procedures. If compliance issues are found while conducting the assessment, Heart & Soul will correct the compliance issue as soon as possible. If the DBHDS licensing specialist identifies and cites policy violations, Heart & Soul will submit a corrective action plan (CAP). Once the CAP is approved by the licensing specialist, Heart & Soul will fully implement and continue monitoring and evaluating progress toward meeting established goals and objectives. Heart & Soul shall continue implementing the corrective action plan and utilize additional measures to prevent the cited violation’s recurrence. The plan will address identified systemic deficiencies when reviews determine that corrective action was fully implemented but did not prevent the cited violation’s recurrence or correct a systemic deficiency according to 12VAC35-105-170. The Quality improvement plan shall be updated to reflect progress or lack of progress on goals.
